Position Overview
Title: Case Manager
Division: Foster Care and Family Services
Location: New Bedford, MA
Schedule: Monday-Friday, 8:30 AM - 4:30 PM (some evening/weekend availability may be required)
Responsibilities
Assess child, youth, and family strengths and needs
Develop supportive relationships with assigned individuals
Participate in team meetings (Treatment Plans, IEPs, Foster Care Reviews)
Provide in-home case management services
Advocate for children and families
Support foster and biological families
Collaborate with internal and external providers
Monitor placements per DCF/EEC/KDC regulations
Maintain documentation and compliance
Transport children/families as needed
Qualifications
Bachelor's degree in Social Work or related field (Psychology, Human Services)
Valid driver's license and reliable vehicle
Strong organizational and communication skills
Bilingual (Portuguese or Spanish) preferred
